China - Post Placement
China Center for Children"s Welfare and Adoption (CCCWA) requires post placement reports to be completed at 1, 6, 12 months after adoption as well as 2, 3, and 5 years after adoption. These reports are to be completed by a Hague accredited agency. The format and forms for post placement reports will be sent to the family and agency once a child is placed in your home. If you have not received this information please contact Ky Bower directly at: ky@adoptionadvocates.org to request this information. The reports must include 8 photos of the child's life.
In addition to the CCCWA requirements, AAI requires family reports. These reports are to be written by your family and submitted directly to AAI. Please use the following format to complete the family reports. Again, these reports are in addition to the agency reports:
